- A copy of your resale certificate issued by your state
- A copy of the owner's driver license. Name must match on the resale certificate
- And if you are located in Texas, a filled out copy of form 01-339.
- For resale fill out page 1.
- For non-profit fill out page 2.

- First order must be over $400 to open a wholesale account.
- $1,500 minimum for free shipping for Texas, Louisiana, and Arkansas; $3,500 for all other states.
- UPC list is currently not available and this website is our only catalog.
